As consideration for the Motherless Daughters Ministry, Inc.. permitting me to participate in a scheduled Motherless Daughters Ministry event, I agree to the following:
ASSUMPTION OF THE RISKS:
I understand that naturally occurring disease processes (including, but not limited to, the novel coronavirus or COVID-19), may exist in the community, including the Motherless Daughters Ministry event and the host facility, and present an inherent risk that cannot be eliminated. I acknowledge that, although Motherless Daughters Ministry and the host facility is taking reasonable measures to avoid contact, transmittal, and exposure of disease processes like COVID-19 between people at the Motherless Daughters Ministry event and the host facility (including between guests, Motherless Daughters Ministry volunteers or employees, host facility employees, and third-parties), it is ultimately my responsibility to ensure that I take appropriate actions to safe-guard myself. I understand and agree that as a guest at Motherless Daughters Ministry event, I am accepting and voluntarily assuming the risk that I may be exposed and become ill as a result of COVID-19 and other diseases.
WAIVER OF LIABILITY:
I, on behalf of myself hereby voluntarily release, forever discharge, and covenant not to sue the Motherless Daughters Ministry, Inc. or the host facility, and their owners, managers, members, employees, agents, and volunteers (“Released Parties”) for any claims that may arise out of or relate in any way to my exposure to any communicable disease, including but not limited to the novel coronavirus or COVID-19. The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties.
I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS. BY REGISTERING FOR AND ATTENDING A MOTHERLESS DAUGHTERS MINISTRY EVENT AT A HOST FACILITY, I UNDERSTAND THAT I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT.
Each party agrees that this Agreement and any other documents to be delivered in connection herewith may be electronically signed and/or initialed, and that any electronic signatures and/or initials appearing on this Agreement or such other documents are the same as handwritten signatures and/or initials for the purposes of validity, enforceability, and admissibility.
